Notice of Privacy Practices for HIPAA Protected Health Information
Your health information is personal and private. It is also known as Protected Health
information (PHI). Recently the federal government put in place new privacy rules that impact
how this Plan handles your PHI.
Your new health care network is ChoiceCare
Effective May 1, 2007, the new health care network is ChoiceCare. Many of the physicians
and hospitals you access now will remain in the ChoiceCare network, more participating physicians
and hospitals are now available.

If you have or are going to have a mastectomy, you may be entitled to certain benefits under
the Women's Health and Cancer Rights Act of 1998 (WHCRA).
New Prescription Benefit Manager - Caremark
Effective June 1, 2007, Caremark will manage your prescription drug benefits.
You can expect to receive your new drug card prior to June 1, 2007.
Retiree, New Prescription Benefit Manager - Caremark
Effective June 1, 2007, Caremark will manage retiree prescription drug benefits.
You can expect to receive your new drug card prior to June 1, 2007.
New Preferred Provider Organization (PPO)
Effective May 1, 2007, the new PPO is Choice Care a leased network from Humana
a national PPO. Many of the physicians and hospitals you access now remain in
the Choice Care network. More physicians and hospitals have been added. Those
of you in the state of Mississippi, specific to Hinds-Rankin-Madison-Lincoln
and Pike counties now have access to more in network providers.
